​Responsible for overseeing day to day patient care, supervising, directing and developing nurse staff to ensure quality patient care within skilled nursing facility.

RESPONSIBILITIES:

• Utilizing expertise in the leadership role as a manager of an assigned unit
• Collaborating with the Director of Nursing in maintaining adequate Nursing coverage to provide safe Nursing care for 24 hours a day, 7 days a week
• Developing, implementing and evaluating protocols/standards of assigned unit while following the proper approval channels
• Assessing the work performance of Nursing personnel
• Assuming Nursing administrative responsibilities of the Center for weekend and holiday coverage as requested

REQUIREMENTS:

• New York RN license
• Up-to-date physical assessment skills
• Comprehensive knowledge of nursing principles
• 1+ year experience in leadership (i.e., Head Nurse, Supervisor, Unit Manger etc.) 
• CPR certification

The social worker will work with residents in the nursing home by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs.

RESPONSIBILITIES:

• Review facility policies and procedures as part of the facility’s interdisciplinary team to assure compliance with state and federal regulations
• Participate in reviewing and setting policies concerning resident care and quality of life
• Participate in developing facility social work policies
• Understand and meet all government requirements for social service documentation
• Document progress in meeting the psychosocial needs of residents
• Work with the interdisciplinary team and administration to promote and protect resident rights and the psychological well-being of each resident.
• Prevent and address resident abuse as mandated by law and professional licensure
• Identify community changes and opportunities such as legislation, regulations and programs that affect nursing home residents
• Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues of the resident, the goals to be accomplished for those needs/issues, and the appropriate social worker interventions
• Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long-term care facility, including their social, emotional, and psychological needs
• Ensure or provide support and education to residents/family members/significant others to assist in their understanding of placement and facility issues in addition to referring them to the appropriate social service agencies when the facility does not provide the needed services.
• Provide groups for residents/family members/significant others as appropriate to their needs
• Provide clinical interventions to address catastrophic events that occur during the resident’s stay in the facility
• Coordinate the resident discharge planning process and make referrals for appropriate home care services prior to the resident’s return to the community
• Educate residents and families/significant others regarding their rights and responsibilities, effective problem solving and the extent of community, health and social services that are available to them, including those necessary for effective discharge planning.
• Review grievance log with Administrator on a monthly basis so he may sign-off on resolved grievances while investigating any open grievances

QUALIFICATIONS:

• LSW, LCSW, or LICSW in the state employed
• Prior work with elders in a community or long-term care setting is preferred
• Must have a working understanding of social systems along with the ability to implement pertinent state and federal regulations.
  

Supervises the activities in the kitchen to provide the highest quality of service and product to residents in a cost-effective manner.
 

RESPONSIBILITIES:

• Directs and supervises all dietary functions and personnel.

• Follows policies and procedures for the Nutritional Services Department.

• Accountable for the proper storage of all food and supplies used by the Nutritional Services Department and minimizing loss.

• Ensures that the kitchen staff follows the required food handling and preparation policies, procedures, and regulations.

• Schedules work hours and assignments also reviews and checks staff work performance. Is responsible to see that each shift is properly staffed.

• Is responsible for food purchase, production, and timely service of meals.

• Evaluates and provides direction in food production, including standardizing recipes, portion control, problem-solving, and Quality Assessment and Assurance activities.

QUALIFICATIONS:

• Certified Dietary Manager (CDM) Certificate

• Long-term Care or Healthcare experience is a plus!

Central Supply is responsible for providing supplies and equipment in an efficient manner, in accordance with federal, state, and local standards, guidelines and regulations

RESPONSIBILITIES:

• Check supply areas daily and stocks with required items.
• Fill requisitions from the nursing department for routine supplies and equipment in accordance with established policies and procedures.
• Ensure facility has adequate emergency supplies.
• Maintain supply and storage rooms in an organized, clean and safe condition in compliance with applicable local, state or federal regulations.
• Ensure inventory is updated as required, when receiving/issuing supplies and equipment. Rotates supplies to avoid loss of expired items.
• Ensure all purchase orders are properly prepared signed and submitted as required in procurement policies and procedures.
• Coordinate claims on damaged merchandise, errors in shipment, etc. and returns to vendor as required.
• Follow process to ensure supplies are tracked and charged appropriately.
• Checks invoices, purchase orders, packing slips, etc., against incoming merchandise.
• Ensures defective equipment is identified for maintenance repair as required in accordance with established policies and procedures.
• Cleans and checks equipment as directed/necessary.
• Provides clerical support to nursing department.

QUALIFICATIONS:

• Must have prior Central Supply experience

SUMMARY:

The RN Nursing Supervisor participates in the direct care and services provided to the residents and is responsible for the quality of nursing care and management of residents for their assigned shift.

JOB DUTIES:

• Manages all charge nurses and CNAs during the shift.
• Develops and promotes communication between the staff including but not limited to unusual occurrences, changes in the routine, or condition of residents.
• Ensures that nursing 24-hour reports are complete and timely.
• Ensures that nurse aides receive accurate and complete assignments with a report at the start of each shift.
• Evaluate residents’ needs for restorative nursing and implements these procedures.
• Knowledgeable of the facility policies and procedures and ensures these are followed by staff on the assigned unit.
• Hold staff accountable for their performance consistent with personnel policies and procedures.
• Administers performance improvement plans and progressive disciplinary actions as needed.
• Documents all disciplinary actions.
• Participates in employee performance evaluations.

QUALIFICATIONS:

• RN License in the state of New York
• 1+ year of nursing leadership experience (charge nurse, supervisor, lead).
• Experience in a long-term care preferred.

Certified Nursing Assistant (CNA) is responsible for providing direct resident care including nursing/clinical care, personal, nutritional, and restorative care.

DUTIES

• Clean and bathe patients or residents
• Help patients use the restroom
• Maintain patients' hygiene and change bedding
• Turn, reposition, and move patients between wheelchairs, seats, and beds
• Serve meals and help patients eat their food
• Measure vital signs like blood pressure and temperature
• Listen to patient’s health concerns and report details to a RN or Doctor
